	Please contact our clinic to verify that your health insurance carrier is accepted by Cardiology Consultants of Texas before your visit. This will assure that your paperwork will go smoothly since patients' plans change over time. As a courtesy, we will file all insurance claims; however, please provide all insurance information and a completed claim form at the time of service. Also understand that your insurance policy is a contract between you and your company. We are not a party to that contract, and therefore, the balance is your responsibility whether your insurance company pays or not. Should you elect to file your own insurance claims, you will be responsible for all charges at the time of service.
