This will assure that your paperwork will go smoothly since patients' plans change over time. As of January 1, 2009, CCT no longer accepts new patients with Medicare Replacements plans.
This list is provided as a courtesy to our CCT patients and may not be fully inclusive of all the plans we accept or don't accept. Since plans change frequently, we ask that you call our office to verify your insurance carrier before your visit. This includes plans shown that we accept, plans that we don't accept, and plans not listed here on our website.As a courtesy, we will file all insurance claims; however, please provide all insurance information and a completed claim form at the time of service. Also understand that your insurance policy is a contract between you and your company. We are not a party to that contract, and therefore, the balance is your responsibility whether your insurance company pays or not. Should you elect to file your own insurance claims, you will be responsible for all charges at the time of service.
